This is FREE and mandatory for all new patients!
Intake appointments are free and take 15 minutes. You will need to have a Photo ID and your Insurance Card.
If you are planning to be a Self-Pay patient, we will need a Credit Card on file.
Kelly will provide and discuss on-boarding new patient forms.
